Aethera Performance Lab is for people whose work or sport depends on how well their body and brain hold up under load — executives, founders, surgeons, trial lawyers, competitive and masters athletes, and parents carrying a heavy week. It runs as two layers that stay deliberately separate.

What this division is for

The wellness layer is Fulcrum Performance Lab's MB8 platform and the Morris Method, licensed from Fulcrum: a baseline assessment, the Fulcrum Score, the Neurological Twin, adaptive programming, breathing protocols, movement and cognitive screens, and hands-on recovery sessions. Fulcrum does not diagnose or treat, and nothing in that layer is medical care. The medical layer is Aethera: a licensed clinician reviews your labs, your exercise test, your body composition and your history, and treats what is actually wrong — low testosterone, insulin resistance, iron deficiency, untreated sleep apnea, a thyroid problem the performance score cannot see.

You can buy either layer alone. The point of buying both is that the clinician and the coach are reading the same measurements, and the person building your training week knows what your bloodwork says. Online in all fifty states.

HSA and FSA work for some of the medical items and not for the wellness ones, and the site says which is which.

What is different here

  • There is a licensed clinician in the loop, and the coach can see what the clinician sees. Fulcrum's own site says plainly that it does not diagnose or treat. Equinox Optimize pairs training with a lab panel but the training staff are not clinicians. This division is the only version of the offer where one record carries both halves.
  • The two layers are visibly separate, and that is the selling point rather than a compliance cost. Two consents, two invoices, two scopes, said out loud on the page. A sophisticated buyer reads that as competence; a regulator reads it as a defence.
  • An evidence page that grades our own menu, including the items that grade badly. Fulcrum's site publishes no citations at all. Saying that a 1.3 ATA chamber is cleared for acute mountain sickness and nothing else, on our own services page, is a credibility asset no competitor in this category is willing to buy.
  • A supply-chain policy instead of a peptide menu. Competitors in the performance space list BPC-157, CJC-1295, ipamorelin and compounded GLP-1s. We publish why we do not, and route the same clinical need to approved products. That is the one position in this market that survives an FDA warning-letter wave.
  • The sleep apnea and iron-deficiency path is at the front of the funnel, not the back. The most common fixable cause of poor performance in a 45-year-old executive is untreated sleep-disordered breathing, and almost nobody in this category tests for it first.
  • Online in all fifty states with enrolment to the medical team at signup, so there is a clinician already assigned when results land, rather than a dashboard and a prompt to book something.

Fulcrum Assessment

A 60-minute in-person baseline session that measures how your nervous system responds to stress and whether it resets, then plots the result on your Neurological Twin with a Fulcrum Score. Delivered by Fulcrum Performance Lab.

A licensed clinician takes your history, orders labs, reviews your exercise test and body composition, and writes a plan. This is the medical half of the program and it is separate from the Fulcrum Assessment.

Who it is for. Anyone buying a medical program, and anyone whose Fulcrum results suggest something a coach cannot address.

A graded exercise test on a treadmill or bike with gas exchange analysis, producing VO2max, ventilatory thresholds and a cardiopulmonary read by a physician. This is the clinical version, not the fitness-studio version.

How you are seen

Residents of every U.S. state and the District of Columbia can join the online clinic. A specific test or treatment can still have its own state limits, and we show any limit that applies in your state before you pay for it.

Concierge visits are available in 48 states. We do not offer concierge care in Massachusetts or New York. The online clinic is open in both.

No Aethera clinic is open yet. Three are in development in Nashville, Miami and Madison, Wisconsin, and a fourth is planned for Bogotá in early 2027. We will say so plainly here the day one opens, and not before.

We are cash-pay. We do not bill insurance, and no federal healthcare program money is involved in anything we do. Many of our services can be paid for with an HSA or FSA card, and some cannot. Eligibility is set by IRS rules and decided by your plan administrator, not by us. Cosmetic treatments are specifically excluded. We mark each service with what we understand its status to be, we give you an itemised receipt with the codes your administrator needs, and we will write a Letter of Medical Necessity where one genuinely applies.
